Below is the link to the forms for this year's PK-12 Flu Shot Clinic.  The permission form is included should you need to print another copy.

The clinic is planned for Friday, October 3th.  Please have all forms in by Thursday, October 2nd.

RSU 50 offers a fluoride varnish program PK-12. This program is funded by Maine's Oral Health Program and offers children in our school the opportunity for fluoride treatment. This is not a mandatory program, but participation is encouraged.

Fluoride helps to strengthen teeth and prevent tooth decay. Fluoride varnish is "painted" onto the teeth and can be brushed off that night or the next morning. Silver Diamine Fluoride (SDF) may also be  applied. This stops a cavity from getting bigger, This may be done twice during the school year,
